Although this monograph was written over 20 years ago, requests for it still come in. It is out of print or distribution through the Centre for Health Promotion at the University of Toronto (which no longer exists), but a PDF file of the document has been made and is free for the downloading. Although some of the arguments or data this monograph presents will be dated, the basic logic of its health promotion practice issues stands up pretty well.
